What Are the Best Sleeping Positions for Back and Neck Pain?

You spend roughly a third of your life asleep — which means your sleep position is quietly doing more for (or against) your spine than almost anything else you do. If you wake with a stiff neck, an aching lower back, or that vague "I slept wrong" feeling, your sleeping posture and pillow setup are the first things worth fixing.

Why Sleep Position Matters
Overnight, your spine should be able to rest in or near its natural curves. Try to avoid: sleeping on your stomach. It forces the neck into rotation and flattens the lumbar curve. If you simply can't change the habit, place a thin pillow under your pelvis to reduce the arch in your lower back, and skip the head pillow or use the flattest one you own.
Best Positions for Neck Pain
Whichever position you sleep in, the neck needs support in line with the rest of your spine:
- Back sleepers: a pillow with medium loft that supports the curve of the neck without pushing the chin toward the chest. A small rolled towel under the neck can help.
- Side sleepers: a pillow tall enough to fill the gap between your ear and the mattress, so your nose stays in line with your breastbone. Too flat and your head tilts down; too high and it tilts up.
- Everyone: avoid stacking pillows. Two or three piled up push the head forward all night — a classic driver of morning neck pain and headaches.
